Connective tissue graft using tunneling technique for root coverage

Gingival recession is a common issue that can cause aesthetic and oral health impacts, leading to exposure of the tooth roots. Surgical treatment is often necessary in severe cases, and the tunneling technique associated with connective tissue grafting has proven effective in these instances. This technique, developed by Raetzke in 1985 and adapted by Allen in 1994, involves creating a tunnel in the soft tissues around the exposed root area and inserting a connective tissue graft to cover the affected area. The tunneling technique is minimally invasive, preserving periodontal structures and reducing post-operative morbidity. However, challenges such as procedure complexity and the risk of nerve and adjacent tissue damage exist. Additionally, gingival recession relapse and limitations in cases of deep recessions are possible. To achieve satisfactory results with tunneling, considering the individuality of each case is essential, taking into account factors like aesthetic demand, presence of keratinized mucosa, and periodontal flap thickness. Overall, the tunneling technique combined with connective tissue grafting is considered the gold standard for treating gingival recessions, providing predictable and long-lasting outcomes when performed by qualified professionals with appropriate post-operative care.
